The disposal of controlled substances at IUPUI is the responisbility of the licensed registrant. Controlled substance registrants have two disposal options available for expired or waste controlled substances.
Reverse Distribution
Indiana University is currently under contract with MedAssure for regulated medical waste disposal services which includes reverse distribution services for controlled substances. Additional information on services provided by visiting the MedAssure website.
On-Campus Incineration
By the expressed approval of the U.S. Drug Enforcement Administration (DEA), campus registrants may dispose of controlled substance through an incinerator owned and operated by the University. There is no charge for this service.
The incinerator operates on an average of once every two months. Registrants in need of disposal services between incineration cycles are to use the reverse distribution option. Under no circumstances are controlled substances to be discarded by flushing the drug down a campus drain.
This disposal option applies to all controlled substances with the exception of chloral hydrate. Contact Environmental Health and Safety at 317-274-4351 for additional guidance with chloral hydrate.
Following are the required steps for this disposal option:

Complete a DEA Form 41 for the inventory of controlled substances to be discarded.

Upload the completed Form 41 to Environmental Health and Safety (EHS) for review.

Upon review of the Form 41, the Registrant will be contacted by a representative of EHS who will schedule a date and time for final destruction of the inventory specified on the Form 41.

The Registrant is responsible for transporting the material to the Campus Services 4 Building located at 1830 W. 16th St. at the agreed-upon date and time.

Under no circumstance will Environmental Health and Safety and/or the Office of Radiation Safety take possession or store any material intended for disposal.

The material will be directly fed into the incinerator under the witness of the Registrant.

Following incineration of the material, the Registrant and a representative of the IUPUI Environmental Health Safety/Radiation Safety and an appropriate law enforcement officer (an Indiana University Police Officer or Officer of another recognized police jurisdiction) will sign the Form 41.

The Registrant will retain the original copy of the Form 41 and will make the form available for inspection by US DEA representatives upon request.

IUPUI Environmental Health and Safety will retain on file a copy of the completed Form 41 following incineration.

In the event a registrant fails to appear at the time of the scheduled destruction, the fact will be clearly documented on/with the Form 41 and retained on file.
